Is it time for home care?

CHHC Goal is to assist our clients with the ability to continue to age in place and maintain their independence.

U.S. Centers for Disease Control and Prevention defines aging in place as “the ability to live in one’s own home and community safely, independently, and comfortably, regardless of age, income, or ability level.” Aging in place endorses life fulfillment, a positive quality of life, and self-confidence. All needed to remain happy, healthy, and well into old age.

  • Bathing
  • Dressing
  • Toileting
  • Transferring
  • Laundry
  • Errands (Additional cost)
  • Hygiene
  • Grooming
  • Companionship

Companion Care
Companion caregivers provide emotional support, assistance with daily tasks, light housekeeping, and companionship to help senior adults maintain independence in their homes.

Emotional Support
Gives residents a sense of belonging and as they age. Research shows the lack of emotional support increases the risk of isolation in the senior community.

Social Engagement
Companions are skilled and apply the right amount of encouragement to keep residents motivated to stay mentally and physically active.

Personal Assistance
Companions can provide light housekeeping, meal preparations, medication reminders, laundry, and assist with many more daily living tasks. A full list of duties will be provided during the initial assessment.

Certified Medical Assistant/Certified Medicine Technician

Medical assistants work in outpatient settings under direct provider supervision of CHHC Director of Nursing and may be delegated clinical and administrative tasks.

Veterans Pension with Aid and Attendance
Assist families across the nation in applying to the VA for an in-home benefit that assists with non-skilled services such as cooking, cleaning, laundry, bathing, dressing and transportation to the grocery store/doctor’s office/pick up RX. CHHC has a Veterans partnership which will coordinate the care associated with the benefit with one of our in-network home care agencies.
Respite
Respite care is the short-term, periodic, and temporary care of individuals with developmental or functional disabilities. It is designed to provide temporary relief for the family or informal caregivers. This type of care gives primary caregivers a break, allowing them to rest, travel, or spend time with other family and friends. Respite care can last anywhere from a few hours to several weeks at a time.
